    FMPC sells ShopRite gift cards as a fundraiser to help support our ministries.  If you would like to purchase a gift card, please fill out the form below. If you would like to purchase more than one gift card, please select from the Choose One section and put the rest of your order in the Comments Box.  Please make all checks payable to FMPC and put ShopRite Gift Cards in the memo.  You can mail your payment to
